Those players who wish to void their registrations to "North Valley Fury" may do so by filling out the Player Request Form and returning it Arizona Fury no later than July 25, 2010.  You may mail the form to Karla Thompson, AZ Fury, 14287 W Desert Cove, Surprise, AZ  85379 or scan and email them to azfurydoc@msn.com.

The Player Request Form requests that "North Valley Fury" void all registrations, agreements, and authorizations made by the requesting player’s parents/guardians and return all such original documents to the player’s parents/guardians at the address indicated on the Player Request Form. 

The Player Request Form also requests that "North Valley Fury" refund all payments made by the requesting player’s parents/guardians.
